CODEX MCP TROUBLESHOOTING CHECKLIST

1. Confirm Codex can see the configured server:
   codex mcp list

2. Review the available MCP commands:
   codex mcp --help

3. Inside the Codex terminal UI, inspect active tools:
   /mcp

4. Check these layers separately:
   - Server name, command or URL
   - Server startup and timeout
   - Authentication
   - Environment variables
   - Tool discovery
   - Tool approval policy

5. Disable unrelated servers and test one connection.

Never paste live credentials into a committed config.toml file.
